Transcript/Records Request

If you are requesting a transcript/records request please fill out the request form. 

 

Upon completing your information on the form, scan your form with a photo id to Jodie Trinh.

 

If you prefer to mail your request form, include a copy of id and mail to:

Requests by mail will be delayed due to current closure.  An e-mail will be sent when the request has been received.
