Required Documents

After completing online enrollment, the following information and materials must be brought to school to finish enrolling your student.

Parent or Guardian Photo Identification
  • A Driver’s License with photo ID or CA ID Card is permitted or,
  • A passport with photo ID

Proof of residency inside SGUSD attendance area
 
Category 1: (Choose One)
• Current mortgage statement
• Closing escrow statement
• Property Tax Bill, payment receipts
• Rental contract or lease (Must be current and list ALL occupant’s names; with landlord name, address and phone number)
 
Category 2: (Choose Two)
• Current gas bill or connection/deposit receipt, service contract, statement or payment receipts
• Current Edison bill or connection/deposit receipt, service contract, statement or payment receipts
• Current Water bill or connection/deposit receipt, service contract, statement or payment receipts
• Current refuse/trash bill or connection/deposit receipt, service contract, statement or payment receipts
• Current pay stubs
• Voter registration
• Correspondence from a government agency

Proof of Age/Birth
  • Official Birth Certificate, Hospital Certificate, Baptismal Certificate
  • Current Passport or Immigration Certificate
Proof of Immunizations
Provide complete up-to-date original immunization records stamped and signed by a physician (choose one):
  • Yellow California Immunization Record
  • Doctor Records
  • School Records

Health Examination
A complete physical examination is mandated by the State of California for all students entering the first grade. However, we recommend that the physical examination be completed at the same time as the immunizations for Kindergarten entry.

Dental Examination
California law states that your child must have a dental check-up by May 31 of the first year in public school.

General Health Examination
SGUSD also requires general health information for all new students.

Transcript or Report Card
  • 6th – 8th Grade – Last year or current year report card and/or transcript from the student’s previous school.
  • 9th – 12th Grade – Official transcript from the student’s previous school.

Additional Information
  • For Special Education students: Current or most recent IEP and Assessments
  • For GATE (gifted and talented) students: Prior test scores and/or certification forms
  • For EL (English Learners) Students, a copy of current CELDT scores, if available.
